Job Description

The British Columbia government is funding the Pharmacists in Primary Care Network (PCN) Program as part of its team-based primary health care strategy. In this program, up to 50 new Primary Care Clinical Pharmacists will be hired by local health authorities and embedded as members of primary health care teams within PCNs across the province. The program officially started in the fall of 2020. The UBC Faculty of Pharmaceutical Sciences is leading the first 3 years of the program in collaboration with health authority and community stakeholders.

Job Summary

The Primary Care Clinical Pharmacist provides clinical pharmacy services, primarily comprehensive medication management to complex patients, while working with one or more medical practice sites within a PCN.

Typical Duties and Responsibilities

• Identify and resolve actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team
• Provide comprehensive medication management, which includes formulating and implementing care plans, monitoring drug therapy outcomes, educating patients and supporting transitions in care
• Maintain appropriate documentation in the health record in accordance with organizational standards and practices as required
• Collaborate with and provide education, consultative and support services to other primary health care team members

Qualifications

• Current registration or ability to be registered as a Pharmacist with the College of Pharmacists of British Columbia
• Minimum 2 years of recent, related patient care experience, ideally in a team-based primary care setting or equivalent combination of education, training and experience
• Valid Class 5 driver’s license and access to a personal vehicle for business related purposes
