Looking for that special pharmacist interested in a team approach for patient centered care. Duties include collaborating with nursing staff in supportive living facilities, individual care plans, PACMED pouching, and building professional relationships.

No evenings or weekends (except for occasional on call service). Flexible schedule. New grads/provisional pharmacists’ welcome.
